Pension Application Forms

Pension Application Form: For applying for pension upon retirement

Family Pension Application Form: For applying for family pension

Nomination Forms: For nominating beneficiaries for pension, DCRG, and family pension

Last Pay Certificate: Certificate of last pay drawn

Non-Drawal Certificate: Certificate for drawing first pension from Treasury

Life Certificate: Annual life certificate required for continuation of pension

Change of Address Form: For updating pensioner's address

Change of Bank Account Form: For changing pension payment bank account

Commutation Application: For applying for commutation of pension
