GPF Forms

The following forms are available for GPF-related applications:

• Form OTC80A: Application for final payment of balances in the Provident Fund account.
• Form OTC80C: Application for final payment by nominees or claimants when no nomination exists.
• Form GPF Advance: Application for temporary withdrawal from the GPF.

These forms can be downloaded from the CAG Odisha website or obtained from the concerned office.
